Determination of renal function with glomerular filtration rate parameter in ambulatory practice

Aim: To study the experience of assessment of renal function using the index of glomerular filtration rate in actual out-patient practice.Materials and Methods: The selected results of laboratory studies of patients on the basis of "Clinical and Diagnostic Center", sought medical help for January 2012. included 343 patients, including 65% of women and 35% men, mean age of 55,9 ± 0,5 years. The patients were divided by the value of GFR in two groups: Group 1 - GFR 60 ml/min/1, 73 m2, Group 2 - GFR less than 60 mL/min/1 73 m2.Results. The structure of patients with CKD revealed a higher prevalence of cardiovascular disease and dyslipidemia than patients without CKD. Established independent predictors of CKD: age older than 70 years, presence of hypertension, coronary heart disease, atherosclerosis of the lower limbs, kidney disease, total cholesterol above 5.0 mmol / l. The population of patients with CKD, masquerading behind the normal value of serum creatinine, 12.6% of the total number of patients studied. In this group of patients age, the structure and the prevalence of comorbidities and dyslipidemia consistent with those in the group of patients with chronic kidney disease and differed from the group of patients without CKD.Conclusion. We consider it appropriate introduction in outpatient clinics estimation of glomerular filtration rate using the MDRD formula as a routine method of diagnosis of renal dysfunction for timely detection of CKD and prescribing treatment aimed at slowing the progression of cardiorenal continuum in these patients.
