Hyperuricemia in chronic renal failure. Cardio-renal effects of allopurinol

Aim. To study the prevalence and severity of hyperuricemia in patients with systolic heart failure and its relationship with clinical, laboratory and hemodynamic parameters to evaluate the effectiveness of the corrective therapy with allopurinol.
Methods. The study included 215 patients with NYHA functional classes II-IV with systolic dysfunction. They were divided into two groups, depending on the presence or absence of hyperuricemia (serum uric acid levels ≥ 360 mmol / l in women and ≥ 420 mmol / l in men) were. Clinico-demographic parameters, as well as laboratory and echocardiographic data were considered. 24 patients received allopurinol 400 mg daily for 3 months, the control group included 22 patients. Results. 60.5% patients had hyperuricemia, which is associated with elevated serum creatinine (p <0,001), urea (p <0,001), NT-pro BNP (p = 0,02), CRP (p <0,001), as well as with decreased GFR after MDRD (p <0,001) and distance covered at 6-MWT (p <0,001), more severe NYHA class and lower ejection fraction.
Conclusion. Hyperuricemia is associated with progression of cardio-renal syndrome in chronic renal failure.
